As students of Boltz's Family Martial Arts Academy, we strive to live by the tenets of Taekwondo; our code of conduct. We believe it is important to receive feedback about our student's efforts. An evaluation must be completed by our student's parent/guardian and a school teacher. We ask that you consider the student requesting this evaluation and determine if you believe the student "HAS" or "HAS NOT" attempted to improve and develop in the areas of Courtesy, Integrity, Perseverance, Self-Control, and having an Indomitable Sprit. We thank you for your assistance and value any extra comments you would like to include in the comments section below. Please do not hold forms as awaiting improvements. Please choose "HAS" or "HAS NOT" as it applies to the student today.

The Philosophy of HAIDONG GUMDO

The philosophy of Haidong Gumdo is to develop one's mind through courage, judgement, respect and temperance; develop one's body through power, speed, balance and sweat; and develop one's spirit through loyalty, filial piety, propriety, and justice.

Teacher Information

Teacher should be a K-12 school teacher, NOT your martial arts instructor. For students with more than one teacher you can chose any teacher you want.

I authorize The Gym, Boltz's Family Martial Arts Academy to charge the test fee to my credit card or bank account on file.
A 3.75% processing fee is charged on all credit card transactions. The Gym: Boltz’s Family Martial Arts accepts check or cash payments with no processing fees.

Alternate Test Date Request
