AUTO-PAY AUTHORIZATION
I authorize The Gym/Boltz's Family Martial Arts to auto-debit my credit card for the amount agreed upon based on my choices above.
Weekly charges will be processed on the first day of each camp week I have registered my child for as indicated on this form.
3 Pay charges will be charged on the same day of each of the 3 consecutive months.
A 3.75% processing fee is charged on all credit card transactions. The Gym: Boltz’s Family Martial Arts accepts check or cash payments with no processing fees.
Payments must be received a minimum of 1 week before due date. A credit card must be on file for all payments in case of a missed check/cash payment to maintain enrollment.
I understand that signing/authorizing below waives my deposit requirement and holds me responsible to pay in-full, minus any early registration discounts, for each of the weeks (full time or part time) of 2024 Summer Camp for which I have registered, regardless of my child's attendance.
